Lake Worth ISD School Enrollment Form (Kindergarten)
by Denise Malone
Child’s Name: My oldest daughter named her. I found out fairly quickly that my oldest had no interest in having a little sister, so I had to figure out a way to create a bond between them. I figured, what better bond than to have her big sister name her. This was the best idea I could have ever had. Her sister named her Heaven Nevaeh. She is our twice-blessed bundle of joy from Heaven.
Address: She has lived at her current address all of her life; five years and three months.
Phone Number: She has not memorized her phone number yet, but we are working on it everyday.
Email Address: I do not believe she is old enough to have an email address and if she did, she still learning to read and write. Maybe in a few more years she will have one, just not at this time. But you can always reach me at my email address if you have concerns or questions for her.
Age: Old enough to talk, walk, and use the bathroom on her own.
Race: She is very fast and most often walks and or runs on her toes. Her pediatricians says this is completely normal, as long as she is able to also walk on the flats of her feet.
Ethnicity: Well I’m African American and her father is Caucasian. So I would imagine she is somewhere in between. If you ask her, she will almost always tell you she is French vanilla.
What language is spoken in the home? We all speak English, but I’m sure my kids think I’m speaking Japanese most of the time as they never listen or do what I say. Don’t get me wrong, there are wonderful kids, but I find myself repeating most of what I tell them on a routine basis.
Parent Contact: My daughter has daily contact with me and her siblings. I think it is very important to have a good relationship with my kids, not just through conversation, but also through hugs and kisses. I read some where that a study was done at an orphanage involving infants. A certain number of babies were given daily physical contact (hugs, kisses, comfort) and the other group, were provided the basic necessities such as feedings and diaper changes. The results were sad, as some of the infants who were only given the basic care passed away. This is why it’s important to me to provide as much contact with my children as possible.
Emergency Contact. Please list at least three people we can contact if you are not available: As a stay at home mom, I am always available. Feel free to contact her Grandmother Liz Everett, her Aunty Shawna, and her Uncle Bob. Just know that if an emergency does arise, they will all contact me. So save yourself some time and so just call me.
Does your child have allergies? Yes, so please do not offer her food items containing lactose or medicines containing any forms of penicillin. Also, please direct emergency personnel to her medical bracelet located on her left wrist for further instruction. I have provided paperwork in the past regarding her allergies and it was completely ignored and or lost.
Does your child have an Epi-pen: No.
Does your child have insurance? Yes. Gerber life, Tricare Prime, and Aetna. But she is not the policy holder on any of these, as she is to young to make any legal and or medical decisions for her self.
Is there anything you would like us to know about your child? Well, I thought that I had provided enough information in previous questions. But if there is more that you would like to know, she is old enough to express her self and answer any other questions you may ask. She may seem shy at first, but once she gets to know you, she will warm up to you very quickly. At that point, she will tell about all of her favorite animals, toys, and her best friends.
Do you give permission for you child’s picture to be taken for classroom projects and/or field trips? I give permission for her picture to be taken. She loves the camera and loves to smile.
